Your rewards are good for a full year. Funds expire 90 days after termination of insurance coverage or 365 days after date rewards were earned, whichever comes first.

The Annual Wellness Visit is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. It is not a head-to-toe physical.
Complete a follow-up visit with a mental health practitioner after being discharged from an acute inpatient hospitalization that included a mental health diagnosis. The follow-up must occur within 30 days of being discharged.
Your reward will depend upon the type of screening your doctor recommends. Only one test per year will be rewarded. The various screening types and rewards available are as follows:

*This card is limited to qualifying products and services as listed above. Eligible items up to the amount of your card balance will be covered. Any remaining balance will remain on your card.
