Using Your Rewards

As you earn money, it'll be added to your new Visa ® Prepaid Card. You'll receive your card by mail after you complete your first screening or care visit.

Remember to keep your Visa ® Prepaid Card after you use it. As you earn more money, it'll automatically be added to your card.

Your rewards are good for a full year. Funds expire 90 days after termination of insurance coverage or 365 days after date rewards were earned, whichever comes first.

All rewards are limited to one per calendar year.

Flu Vaccine

Vaccine must be received during flu season (September - March).

Ongoing Diabetes Care

Rewards can be earned for completing an HbA1c test, kidney screening (urine protein test), and retinal (eye) screening.

Annual Wellness Visit

The Annual Wellness Visit is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. It is not a head-to-toe physical.

Follow-Up Visit after Hospitalization with Mental Health Diagnosis

Complete a follow-up visit with a mental health practitioner after being discharged from an acute inpatient hospitalization that included a mental health diagnosis. The follow-up must occur within 30 days of being discharged.

Colon Cancer Screening

Your reward will depend upon the type of screening your doctor recommends. Only one test per year will be rewarded. The various screening types and rewards available are as follows:
